I could reproduce the issue in my internal system. First of all, your template must have the first row as blank. The M letter (first char of the row) indicates that the information on that row is a header. There is a table on your template which does not have a header. Also, to reproduce the issue in my internal system, I had to erase the blank row (first row of the document) and erase the table header (M row).
Be sure your template is similar to this one below. Also, make sure all the tables have their headers rows.
