Human failure.
A physical inventory does not create itself and it does not close itself. So it was created by somebody and then forgotten. This can be a lost counting sheet or maybe even error messages when posting counts or differences and putting it aside for later and again: forgotten.
And monitoring obviously is not included in your physical inventory activities. You really need to add this step to make sure that all physical inventory documents are posted at the end of a counting day.
