I have the same issue! I thought it was a fluke. The employee isn't getting two 1095s and isn't counted twice in the 1094. The same employee has been coming up twice in HRBENUS_ACA_EVAL since January testing. We assumed it had something to do with master data, but couldn't identify what is different about this one employee vs. others. I haven't opened an incident for this yet.
